It is advisable to review the policy (or contract) of the annuity and/or annuity termination or enrollment procedures to ensure they are consistent with the federal regulations.  Annual Annuity Termination/Enrollment Forms and Brochures: OPM Form 2810, Special Form 260, Annual Annuity Termination: A Social Security Withdrawal from the Retirement System.  Annuitants that elect to terminate their annuity must complete Form 2810 and provide the employer's or spouse's Social Security number (SSN) or date of birth (birthdate)  (as applicable to individual annuities) as proof that they are eligible to terminate the annuity.  On Form 2810, if the annuitant is the only eligible family member, they may select to terminate their annuity prior to open season.
